ComDiab
ComDiab is a community-based education service for low-risk and newly diagnosed diabetic patients. The program aims to ease the burden on hopsital-based diabetes services. Patients attend two 1 hour education sessions run by Community Health Nurses or Practice Nurses and learn practical information to assist them to better understand and self-manage their diabetes. Please click on the ComDiab referral form in the Downloads section below.
