DARTYP: Disability Assessment and Rehabilitation Team for Young People
SLHN (formerly SSWAHS) has recently established a new service at Concord Hospital focusing on young people with disabilities. The service is managed by the centres Rehabilitation Physician Dr Veena Raykar and Clinical Nurse Consultant Kiran Singh, offering comprehensive management of young people with lifelong disabilities to ensure that all health care is coordinated to provide the highest level collaborative care. The team works with patients, families, GPs, health and community services to ensure the transition from pediatric to adult services is a smooth transition.
The DARTYP team included rehabilitation physicians, nurses, occupational therapists, social worker, physiotherapists and speech pathology all at the same centre. Assessments are run weekly with special clinics for spinal bifida, spasticity, transitional care and botox run monthly.
The DARTYP team has offered to supervise CSGPN GPs interested in participating in Clinical Attachments at the centre. They will be run from April to November of this year. GPs will have to undergo hospital clearance checks before attending the clinics. To gain RACGP category 1 40 CPD points GP will need to attend 10 hours of clinics. The clinics will be divided in two hour slots offering either morning or afternoon sessions, five sessions will need to be completed.
