ComDiab
ComDiab provides group introductory diabetes education in the community for any of your patients, carers / family members or anyone who wants to know about diabetes. Facilitated by well trained nurses, the education offers practical information to enable individuals to better understand and self manage diabetes. ComDiab does not replace individual diabetes education services.

What is unique about ComDiab?

  • Free of cost to the public
  • No referral required
  • Readily accessible, especially when delays in accessing other services

Who runs ComDiab?
Community health / practice nurses who are well trained and assessed in diabetes education by the Australian Diabetes Council

How many sessions can each individual get?
Session 1: Overview: What is Diabetes - 2 hours
Session 2: Diabetes Management: Practical Aspects - 2 hours

Where do these sessions take place?
Various locations within the CSGPN catchment area [Croydon, Marrickville, Canterbury, Concord and Redfern]

Who can benefit from this program?
Anyone who wants to know about diabetes, especially those who are at risk of diabetes or newly diagnosed with diabetes

What should my patients / carers do to attend a session?
Please provide your patient with a tear off page from the ComDiab pad or ask them to ring the 1300 342 238 number to book into the group sessions nearest to them.

How do I know whether my patient received the necessary education?
A feedback letter will be sent to you indicating attendance at the sessions.
