Type Two Diabetes Risk Evaluation
Target: people aged 40-49 at risk of developing Type 2 diabetes
Claiming rules
- Once every three years
- Claimed with bulk billing incentive
Can be completed by the
- GP
- GP and practice nurse
The patient
- aged between 40 - 49 years of age Complete AUSDrisk assessment tool
- Have consent from parent or carers
- Be provided with a copy of Get set for Life
- Clinical risk factors
- Lifestyle factors
- Smoking
- Poor nutrition
- inactivity
Biomedical risk factors
- blood pressure
- weight
- impaired glucose metabolism
- recent diagnostic test results
- family history of chronic illness
The health assessment will include
- evaluation of patients risk using AusDRISK completed at least 3months before the health assesment
- physcial examination and update of patients history and clincial investigation
- make the assessment of patients risk
- initiate patients readiness to change interventions
- recommend Lifestyle Modification Program (LMP)
- Contact CSGPN for referral number and provider
- Implement motivational interviewing and lifestyle coaching

- ComDiab is a Sydney South West Integration Program (SSWIP) initiative which aims to provide GPs with an alternative diabetes education referral pathway for people diagnosed with ‘low risk’ type 2 diabetes. Community Health Nurses and/or Practice Nurses deliver basic diabetes education over two 1-hour sessions at several locations around Sydney.
To refer into ComDiab, dowload the referral form or install the Best Practice/Medical Director template onto your practice software. Your patient then calls Community Health Access Intake Nursing (CHAIN) on 1300 722 276 for program times and locations.
Referrals
Referrals to a Lifestyle Modification Program (LMP) are for eligible patients who are aged 40-49 years and have scored 12 or over on the AUSDRISK tool.
To refer, GPs need to
- administer the AUSDRISK tool
- complete either the generic referral form or an electronic referral form
- ring CSGPN on 9799 0933 to get an authorisation number
- fax referral to the division on our secure fax number 9009 0602 and we will pass it onto the nominated provider
